If you live within our practice catchment area we will require you, and each member of your family who wishes to register, to have an appointment with the Primary Health Care Assistant for a new patient health check. Once this is complete you will be required to fill in a Family Doctor Services Registration (GMS1 form) and you will then be fully registered with the practice.
You have the right to express a preference to receive services from a particular doctor/nurse, either for a general problem or particular condition. We will record your preference in your computerised medical record on the day your request was made.
At the time of registration you will be asked for details of your ethnic origin.
