New Patients

If you live within our practice catchment area we will require you, and each member of your family who wishes to register, to have an appointment with the Primary Health Care Assistant for a new patient health check. Once this is complete you will be required to fill in a Family Doctor Services Registration (GMS1 form) and you will then be fully registered with the practice.

You have the right to express a preference to receive services from a particular doctor/nurse, either for a general problem or particular condition. We will record your preference in your computerised medical record on the day your request was made.

At the time of registration you will be asked for details of your ethnic origin.

Exclusions

Patients may still be registered from outside our catchment area if they fall within the below criteria:

If you are living with a patient who is already registered at this practice.
Any Babies whose parents are already registered here at Bridgegate.

At the end of the Registration Process, it is possible that you will be told that you are out of our Catchment Area and you will have the two following options:

Option One – Carry on with your application and when we receive the Registration Request, this could be rejected (For those patients included in our exclusions (as above) carry on with your registration)

Option Two - Cancel the Registration and look for a GP Practice in your Catchment Area.
